Symptom: After using flashupdate to update a system, the next reboot of the domain results in the following error: 

Oct 22 12:58:38 ds2-sc0 Domain-A.POST: ERROR: Failed to decompress Open 
Firmware image ({/N0/SB0/P0}  failed to decompress Open Firmware image)
SOLUTION SUMMARY:

Workaround:

Issue a setkeyswitch off on the domain, and then setkeyswitch on. This will reset the domain.

This problem can be avoided by following the instructions on page 8 of the 5.12.6 Software Release Notes.
