PROBLEM DESCRIPTION:
A host system using the 375-0040-01 or -02 FC-AL PCI card to attach to FC-AL
storage, may experience loop errors which affect data access to that storage.
In extreme cases, this can result in the host losing its connection to the
storage completely. However, it is more common for the loop errors to be
intermittent initially, perhaps becoming more frequent over a period of days
or a few weeks. The failures are sometimes sensitive to the data pattern on
the FC-AL loop, so they are not always reproducible with current diagnostic
software.
Below are examples of error messages;

Root Cause has determined that the Molex optical transceivers on FC100/P cards
(Sun p/n 375-0040-01 and -02) may experience premature failure. To improve
overall product reliability Siemens optical transceiver modules are now used
on the 375-0040-03.
Corrective action was taken in Sun Manufacturing on November 3, 1999 by
releasing the 375-0040-03 via ECO# WO_16166.
Corrective action was initially taken in Enterprise Services on December 20th,
1999 via LEAP_1074 which called for the rework upon failure of the 375-0040-02
FC100/P cards to -03 . Follow up corrective action was taken in Enterprise
Services on March 20th, 2000 via LEAP_1074A and DPCO_182 which called for the
purge and rework of all 375-0040-01 and -02 FC100/P cards to -03.

Visual methods of identifying revision level of FC-AL PCI Cards;
Molex transceiver (defective) has BLACK tabs inside the black SC
connector housing. This transceiver may be found on either the
-01 or -02 boards.
Siemens transceiver (good) has WHITE tabs inside the black SC
connector housing. This transceiver started shipping on the
-03 boards, and will be found on boards reworked to -03.
The optical cable must be pulled out from the FC-AL PCI Card
to order to check tab color.

Upon failure or upon request replace 375-0040-02 (or below) with 375-0040-03.

Warranty: Sun will provide parts at no charge under Warranty
Service. On-Site Labor Rates are based on how the
system was initially installed.
Contract: Sun will provide parts at no charge. On-Site Labor Rates
are based on the type of service contract.
Non Contract: Sun will provide parts at no charge. Installation by
Sun is available based on the On-Site Labor Rates
defined in the Price List.

i) In case of Mandatory FCOs, Enterprise Services will attempt to contact
all known customers to recommend the part upgrade.
ii) For controlled proactive swap FCOs, Enterprise Services mission critical
support teams will initiate proactive swap efforts for their respective
accounts, as required.
iii) For Replace upon Failure FCOs, Enterprise Services partners will implement
the necessary corrective actions as and when they are required.
