PROBLEM DESCRIPTION:

Ultra 5 and Ultra 10 systems which use the 24x Lucky Goldstar CDROM    
drive may fail to boot a Solaris 2.5.1 HW 11/97 CDROM (media).  A    
typical error message displayed on an Ultra 5 or Ultra 10 console 
during an attempt to boot from an affected Solaris 2.5.1 Hardware  
11/97 CDROM may be as follows:


  NOTICE: hs_findhsvol:bread:error=[5]
  hsfs mount failed, trying ufs....
  mount:/devices/pci@1f,0/pci!1,1/ide@3/atapicd@2,0:a is not fs type
  Bad /usr
  Type: hsfs,Path:/devices -------/atapicd@2,0:a


Upon further investigation, the root cause of this problem was traced  
back to a fault with one of the systems that Sun vendor 'Imation' used    
to manufacture the Solaris 2.5.1 Hardware 11/97 CDROM (media).

The only way to identify the suspect Solaris 2.5.1 Hardware 11/97 CDROM    
is to read the CD's center plastic ring.  The inner plastic ring on the  
CDs that have been identified to have problems booting from Ultra 5 and 
Ultra 10 systems have the following information imprinted on the inner 
ring:

	 	Made by Imation CR76321D 971119
 
 
*NOTE: The Solaris 2.5.1 HW 11/97 CD will work fine on all other 
       platforms.  
 

As a result of the above stated problem, stop ship WW001#3528 has been 
issued and inventory is now being replaced with new CDs that have been  
tested and verified.

CORRECTIVE ACTION: 


In order to determine if the media is actually affected you must read 
the inner plastic ring of the Solaris 2.5.1 HW 11/97 CD.  If you have  
the following information on that center ring then, you should have the 
CD replaced:

         		Made by Imation CR76321D 971119


Solaris 2.5.1 HW 11/97 CDs that are NOT affected by the Ultra 5/Ultra 10
boot problem can be identified by verifying the impression on the inner 
ring of the CD which reads:

			Made by Imation NR76321A 971117
		

COMMENTS: 

It was determined that this problem was limited to NAFO distribution 
and should not effect any other GEO's.  Also, this problem can only be 
reproduced on the Ultra 5 and 10 systems that use the 24X Lucky Goldstar
CD-ROM drive.

i)   In case of MANDATORY FINs, Enterprise Services will attempt to contact
     all affected customers to recommend implementation of the FIN.


ii)  For CONTROLLED PROACTIVE FINs, Enterprise Services mission critical sup-
     port teams will recommend implementation of the FIN (to their respective
     accounts), at the convenience of the customer.


iii) For REACTIVE FINs, Enterprise Services will implement the FIN as the need
     arises.
