PROBLEM DESCRIPTION:
A problem exists when one of the power supplies internal fuse opens,
and power leaks from the good power supply to the bad power supply
causing the system to eroneously report both power supplies as good.
On a multiple power supply E450 system, it is possible for power to
leak from one power supply to another as one power supply fails, thus
masking the failure. The system will still function without any
degradation but the redundancy feature is lost. Failure of the
second power supply will result in a system down situation.
Under normal circumstances there would be no error messages or changes
in system behavior. However, if the system is heavily loaded and has
2 or 3 power supplies, one power supply may go down and the other
would not be able to handle the load and would also go down. Upon
manual examination, the Field Service engineer will not see the LED
lit on the power supply that has failed.
Usually there are 2 ways to identify whether the power supply is losing
power, however it is not possible to identify either way in this case.
Case 1:

On the front of the E450 there are 6 status LEDs, beside the keyswitch.
The "wrench" LED is lit if there is a problem with the system, such as
a Power On selftest failure or a failed power supply.
Case 2:
-------
Type 'prtdiag -v' command to check the status of power supply. A
"failed" message should be displayed on the "Status" section.
However
'prtdiag -v' command doesn't detect and display correct power status.

The possible root cause of this problem is a diode on some power supplies
leaks when the DC side of the power supply fails. The DC side of the 2nd
power supply will then leak into the failed power supply. When this occurs,
the system will see the failed power supply as good and will not report
the failure.
This product will be EOLed in 2nd quarter of 2002. Corrective action
was made available by releasing the 300-1302-03 via ECO# WO_10853 on
July 11, 1997 and 300-1359-04 via ECO# WO_14426 on February 2, 1999.

CORRECTIVE ACTION:
The following recommendation is provided as a guideline for authorized
Enterprise Services Field Representatives who may encounter the above
mentioned problem.
Field engineers must periodically visually inspect the status LEDs on
the power supplies located on the back of the E450 systems. If the LEDs
are not lit, this may be a sign that the power supplies are defective.
If this situation is experienced, replace power supply 300-1302-02
(or below) with 300-1302-03 (or above) or power supply 300-1359-03
(or below) with 300-1359-04 (or above).

i) In case of MANDATORY FINs, Enterprise Services will attempt to
contact all affected customers to recommend implementation of
the FIN.
ii) For CONTROLLED PROACTIVE FINs, Enterprise Services mission critical
support teams will recommend implementation of the FIN (to their
respective accounts), at the convenience of the customer.
iii) For REACTIVE FINs, Enterprise Services will implement the FIN as the
need arises.
